Biographies offer readers a unique glimpse into the lives of others. Some biographies are supportive, while others are critical. Some observe a life through a political perspective, while others are told through the context of history and particular circumstances. However the story is told, the biography’s conclusion should offer the readers a sense of closure. If you describe the subject’s legacy, incorporate personal details, and support your main thesis, you can write a successful conclusion for a biography.

Reminding the Reader of the Subject’s Importance

Step 1 Summarize the subject's most memorable actions.

Step 3 Discuss the subject's legacy.

  • A biography of Susan B. Anthony, for example, could mention that her actions and beliefs influenced generations of women and their continued fight for gender equality.

Including Personal Details

Step 1 Humanize the subject.

  • Discuss how they loved visiting their grandmother, or mention that their favorite place was a quiet cabin in the mountains.

  • The late Nelson Mandela led an influential, prominent, and successful life as a South African activist and former president. Although he earned many awards and honors, he admitted that he was not a very good student when he was trying to earn a law degree. Step 3 Describe their hobbies or passions.

  • Weave their personal interests into the conclusion by acknowledging how those interests influenced their work or life decisions.

Bringing It All Together

Step 1 Recap your key points.

  • Briefly remind the audience of the key points that influenced your thesis. Perhaps your subject moved a lot as a child, and you believe the subject’s bestselling novels were influenced by this. Point out that the main character in the novel is often on the go, and that was the basis for your thesis.

Step 2 Refrain from introducing new ideas or details.

  • If you believed that the subject’s actions later in life were a result of their experiences during war, briefly reassert what major decisions you think were affected. Perhaps they participated in anti-war protests, founded an organization that promotes peace, and worked to uphold human rights. Make a note that their actions revolved around establishing peace.
  • Remind the reader what makes the subject unique or special and what life events demonstrate these qualities.

Step 4 Avoid using transitional phrases.

  • Instead of saying, "in conclusion," try to provide your reader with a subtle transition instead. Conclude by saying, "Because of the magic and popularity of her novels, I believe that J.K. Rowling will continue to influence young readers for generations to come."

Step 5 Use direct quotes from your subject if they will strengthen your thesis.

  • This is an exception to the rule of not adding new information to the conclusion, as quotes shouldn’t be repeated throughout the biography.

Step 6 Provide closure.

  • Conclude with a sense of hope or continued progress. Allow your reader to imagine that this person’s life story will continue influence the lives of others. If your subject is still alive, for example, provide a hopeful ending by saying “Elizabeth continues to fight for the rights of those who have been repressed by being a vocal leader in the United States Senate.”

How to write a biography can be a fun challenge as you share someone’s life story with readers. You may need to write a biography for a class or decide to write a biography as a personal project. Once you’ve identified the subject of your biography, do your research to learn as much as you can about them. Then, immerse yourself in writing the biography and revising it until it’s best. What I am going to share with you in today’s post is how to write a biography. If you want to know the rules of how to write a biography correctly then this post of ours is essential for you. 

Introduction

While it’s true that most biographies involve people in the public eye, sometimes the subject is less well-known. But most of the time, famous or not, the person we’re talking about has an incredible life. Although your students may have a basic understanding of How to write a biography, you should take some time before putting pen to paper to come up with a very clear definition of biography.

Before knowing how to write a biography, let’s first understand what a biography is. A biography is an account of a person’s life written by someone else. Although there is a genre called fictional biography, by definition biographies are mostly non-fiction. In general, biographies trace the subject’s life from early childhood to the present day or until death if the subject is deceased. 

Biography writing is not limited to describing the bare facts of a person’s life. Instead of just listing basic details about their upbringing, interests, education, work, relationships, and deaths, a well-written biography should also paint a picture of a person’s personality as well as that person’s life experiences.

Tips and Tricks For How To Write a Biography

1. ask the subject’s permission to write a biography.

Here are the first tips on how to write a biography. Before starting your research, make sure you get your subject’s consent to write their biography. Ask them if they’re ready to be the subject. Getting their permission will make writing a biography much easier and ensure that they are open to information about their lives.

If the theme does not allow you to write a bio, you can choose another theme. If you decide to publish a profile without the subject’s permission, you may be subject to legal action from the subject. 

If the topic no longer exists, you don’t need to ask permission to write about them. 

2. Research primary sources on the topic

Primary sources may include books, letters, photographs, diaries, newspaper clippings, magazines, Internet articles, magazines, videos, interviews, existing biographies, or autobiographies on the subject. Find these resources in your local library or online. Read as much as you can about the topic and highlight any important information you come across in your sources. 

You can create research questions to help you focus your research on this topic, such as: 

What do I find interesting about this topic? Why is this topic important to readers? 

3. Conduct interviews with subjects and their relatives

Interviewing people will turn your research into reality: the people you interview will be able to tell you stories you can’t find in history books. Interview the subject as well as people close to them, such as spouses, friends, business associates, family members, co-workers, and friends. Interview in person, over the phone, or via email.

For in-person interviews, record them with a voice recorder or voice recorder on your computer or phone. You may need to interview the subject and others multiple times to get the documents you need.

4. Visit places important to the topic

Whenever you want to know how to write a biography, to understand the history of the subject, spend time in places and areas that are significant to the subject. This may be the subject’s childhood home or neighborhood. You can also visit the subject’s workplace and regular meeting places. 

You may also want to visit areas where the subject made important decisions or breakthroughs in their life. Being physically present in the area can give you an idea of what your subjects may have felt and help you write about their experiences more effectively.

5. Research the time and place of the subject’s life

Contextualize your subject’s life by observing what’s going on around them. Consider the period in which they grew up as well as the history of the places they lived. Study the economics, politics, and culture of their time. See current events happening where they live or work.

When you studying how to write a biography, ask yourself about time and place: 

What were the social norms of this period? 

What happened economically and politically? 

How has the political and social environment influenced this topic?

6. Make a timeline of a person’s life

To help you organize your research, create a timeline of a person’s entire life, from birth. Draw a long line on a piece of paper and sketch out as many details about a person’s life as possible. Highlight important events or moments on the timeline. Include important dates, locations, and names. 

If you think about how to write a biography You can also include historical events or moments that affect the topic in the timeline. For example, a conflict or civil war may occur during a person’s lifetime and affect their life.

7. Focus on important events and milestones

Major events can include marriage, birth, or death during a person’s lifetime. They may also achieve milestones like their first successful business venture or their first civil rights march. Highlights key moments in a person’s life so readers clearly understand what’s important to that person and how they influence the world around them.

For example, you might focus on one person’s achievements in the civil rights movement. You could write an entire section about their contributions and participation in major civil rights marches in their hometowns.

8. Cite all sources used in  biography

Most biographies will include information from sources such as books, journal articles, magazines, and interviews. Remember to cite any sources that you directly quote or paraphrase. You can use citations, footnotes, or endnotes. If the biography is for a course, use MLA, APA, or Chicago Style citations according to your instructor’s preference.

9. Reread the biography

Check the biography for spelling, grammar, and punctuation. Circle all punctuation marks in the text to confirm they are correct. Read the text backward to check for spelling and grammar errors. 

Having a biography full of spelling, grammar, and punctuation errors can frustrate readers and lead to poor grades if you submit your work to the class.

10. Show your biography to others to get their feedback

It is a momentous step of how to write a biography. Once you have completed your draft biography, show it to your colleagues, friends, teachers, and mentors to get their feedback. Ask them if they have a good understanding of someone’s life and if the biography is easy to read. Be open to feedback so you can improve the biography and make it error-free. Revise profile based on feedback from others. Don’t be afraid to trim or edit your biography to suit your readers’ needs.

11. Use flashbacks

Flashbacks happen when you move from the present to the past. You can start with the present moment, and then bring in a scene from the person’s past. Or you could have one chapter focusing on the present and one focusing on the past, alternating as you go.

The flashback scene must be as detailed and realistic as the present-day scene. Use your research notes and interviews with subjects to better understand their past to reminisce. 

For example, you can move from a person’s death in the present to reminiscing about their favorite childhood memory.

12. Outline Your Story Chronologically 

This is another important step in how to write a biography is to write an outline that describes your story in chronological order. An outline is a tool that helps you visualize the structure and key elements of your story. This can help you organize your story into chapters and sections. 

You can write your plan in a digital document or draw it with pen and paper. Remember to store your outline in an easily accessible place so you can refer to it throughout the writing process.

Use MLA, APA, or Chicago Style citations based on your instructor’s preference when citing sources in your biography.

How to Write a Biography: A 7-Step Guide [+Template]

From time to time, nonfiction authors become so captivated by a particular figure from either the present or the past, that they feel compelled to write an entire book about their life. Whether casting them as heroes or villains, there is an interesting quality in their humanity that compels these authors to revisit their life paths and write their story.

However, portraying someone’s life on paper in a comprehensive and engaging way requires solid preparation. If you’re looking to write a biography yourself, in this post we’ll share a step-by-step blueprint that you can follow. 

How to write a biography: 

1. Seek permission when possible 

2. research your subject thoroughly, 3. do interviews and visit locations, 4. organize your findings, 5. identify a central thesis, 6. write it using narrative elements, 7. get feedback and polish the text.

While you technically don’t need permission to write about public figures (or deceased ones), that doesn't guarantee their legal team won't pursue legal action against you. Author Kitty Kelley was sued by Frank Sinatra before she even started to write His Way , a biography that paints Ol Blue Eyes in a controversial light. (Kelley ended up winning the lawsuit, however).

Whenever feasible, advise the subject’s representatives of your intentions. If all goes according to plan, you’ll get a green light to proceed, or potentially an offer to collaborate. It's a matter of common sense; if someone were to write a book about you, you would likely want to know about it well prior to publication. So, make a sincere effort to reach out to their PR staff to negotiate an agreement or at least a mutual understanding of the scope of your project. 

At the same time, make sure that you still retain editorial control over the project, and not end up writing a puff piece that treats its protagonist like a saint or hero. No biography can ever be entirely objective, but you should always strive for a portrayal that closely aligns with facts and reality.

If you can’t get an answer from your subject, or you’re asked not to proceed forward, you can still accept the potential repercussions and write an unauthorized biography . The “rebellious act” of publishing without consent indeed makes for great marketing, though it’ll likely bring more headaches with it too. 

✋ Please note that, like other nonfiction books, if you intend to release your biography with a publishing house , you can put together a book proposal to send to them before you even write the book. If they like it enough, they might pay you an advance to write it.

Once you’ve settled (or not) the permission part, it’s time to dive deep into your character’s story.  

Deep and thorough research skills are the cornerstone of every biographer worth their salt. To paint a vivid and accurate portrait of someone's life, you’ll have to gather qualitative information from a wide range of reliable sources. 

Start with the information already available, from books on your subject to archival documents, then collect new ones firsthand by interviewing people or traveling to locations. 

Browse the web and library archives

Put your researcher hat on and start consuming any piece on your subject you can find, from their Wikipedia page to news articles, interviews, TV and radio appearances, YouTube videos, podcasts, books, magazines, and any other media outlets they may have been featured in. 

Establish a system to orderly collect the information you find 一 even seemingly insignificant details can prove valuable during the writing process, so be sure to save them. 

Depending on their era, you may find most of the information readily available online, or you may need to search through university libraries for older references.

For his landmark biography of Alexander Hamilton, Ron Chernow spent untold hours at Columbia University’s library , reading through the Hamilton family papers, visiting the New York Historical Society, as well as interviewing the archivist of the New York Stock Exchange, and so on. The research process took years, but it certainly paid off. Chernow discovered that Hamilton created the first five securities originally traded on Wall Street. This finding, among others, revealed his significant contributions to shaping the current American financial and political systems, a legacy previously often overshadowed by other founding fathers. Today Alexander Hamilton is one of the best-selling biographies of all time, and it has become a cultural phenomenon with its own dedicated musical. 

Besides reading documents about your subject, research can help you understand the world that your subject lived in. 

Try to understand their time and social environment

Many biographies show how their protagonists have had a profound impact on society through their philosophical, artistic, or scientific contributions. But at the same time, it’s worth it as a biographer to make an effort to understand how their societal and historical context influenced their life’s path and work.

An interesting example is Stephen Greenblatt’s Will in the World . Finding himself limited by a lack of verified detail surrounding William Shakespeare's personal life, Greenblatt, instead, employs literary interpretation and imaginative reenactments to transport readers back to the Elizabethan era. The result is a vivid (though speculative) depiction of the playwright's life, enriching our understanding of his world.

Many readers enjoy biographies that transport them to a time and place, so exploring a historical period through the lens of a character can be entertaining in its own right. The Diary of Samuel Pepys became a classic not because people were enthralled by his life as an administrator, but rather from his meticulous and vivid documentation of everyday existence during the Restoration period.

Once you’ve gotten your hands on as many secondary sources as you can find, you’ll want to go hunting for stories first-hand from people who are (or were) close to your subject.

With all the material you’ve been through, by now you should already have a pretty good picture of your protagonist. But you’ll surely have some curiosities and missing dots in their character arc to figure out, which you can only get by interviewing primary sources.

Interview friends and associates

This part is more relevant if your subject is contemporary, and you can actually meet up or call with relatives, friends, colleagues, business partners, neighbors, or any other person related to them. 

In writing the popular biography of Steve Jobs, Walter Isaacson interviewed more than one hundred people, including Jobs’s family, colleagues, former college mates, business rivals, and the man himself.

When you conduct your interviews, make sure to record them with high quality audio you can revisit later. Then use tools like Otter.ai or Descript to transcribe them 一 it’ll save you countless hours. 

You can approach the interview with a specific set of questions, or follow your curiosity blindly, trying to uncover revealing stories and anecdotes about your subject. Whatever your method, author and biography editor Tom Bromley suggests that every interviewer arrives prepared, "Show that you’ve done your work. This will help to put the interviewee at ease, and get their best answers.” 

Bromley also places emphasis on the order in which you conduct interviews. “You may want to interview different members of the family or friends first, to get their perspective on something, and then go directly to the main interviewee. You'll be able to use that knowledge to ask sharper, more specific questions.” 

Finally, consider how much time you have with each interviewee. If you only have a 30-minute phone call with an important person, make it count by asking directly the most pressing questions you have. And, if you find a reliable source who is also particularly willing to help, conduct several interviews and ask them, if appropriate, to write a foreword as part of the book’s front matter .

Sometimes an important part of the process is packing your bags, getting on a plane, and personally visiting significant places in your character’s journey.

Visit significant places in their life

A place, whether that’s a city, a rural house, or a bodhi tree, can carry a particular energy that you can only truly experience by being there. In putting the pieces together about someone’s life, it may be useful to go visit where they grew up, or where other significant events of their lives happened. It will be easier to imagine what they experienced, and better tell their story. 

In researching The Lost City of Z , author David Grann embarked on a trek through the Amazon, retracing the steps of British explorer Percy Fawcett. This led Grann to develop new theories about the circumstances surrounding the explorer's disappearance.

Hopefully, you won’t have to deal with jaguars and anacondas to better understand your subject’s environment, but try to walk into their shoes as much as possible. 

Once you’ve researched your character enough, it’s time to put together all the puzzle pieces you collected so far. 

Take the bulk of notes, media, and other documents you’ve collected, and start to give them some order and structure. A simple way to do this is by creating a timeline. 

Create a chronological timeline

It helps to organize your notes chronologically 一 from childhood to the senior years, line up the most significant events of your subject’s life, including dates, places, names and other relevant bits.

You should be able to divide their life into distinct periods, each with their unique events and significance. Based on that, you can start drafting an outline of the narrative you want to create.  

Draft a story outline 

Since a biography entails writing about a person’s entire life, it will have a beginning, a middle, and an end. You can pick where you want to end the story, depending on how consequential the last years of your subject were. But the nature of the work will give you a starting character arc to work with.

While biographies tend to follow a chronological narrative, you can use flashbacks to tell brief stories or anecdotes when appropriate. For example, if you were telling the story of footballer Lionel Messi, before the climax of winning the World Cup with Argentina, you could recall when he was just 13 years old, giving an interview to a local newspaper, expressing his lifelong dream of playing for the national team.

2. Compile detailed descriptions of the following personal traits:

  •      Values and beliefs

3. Compile some research questions based on different topics to provide a focus for the research:

  • Childhood : Where and when were they born? Who were their parents? Who were the other family members? What education did they receive?
  • Obstacles: What challenges did they have to overcome? How did these challenges shape them as individuals?
  • Legacy: What impact did this person have on the world and/or the people around them?
  • Dialogue & Quotes: Dialogue and quotations by and about the subject are a great way to bring color and life to a biography. Students should keep an eagle eye out for the gems that hide amid their sources.

As the student gets deeper into their research, new questions will arise that can further fuel the research process and help to shape the direction the biography will ultimately go in.

Likewise, during the research, themes will often begin to suggest themselves. Exploring these themes is essential to bring depth to biography, but we’ll discuss this later in this article.

Research Skills:

Researching for biography writing is an excellent way for students to hone their research skills in general. Developing good research skills is essential for future academic success. Students will have opportunities to learn how to:

  • Gather relevant information
  • Evaluate different information sources
  • Select suitable information
  • Organize information into a text.

Students will have access to print and online information sources, and, in some cases, they may also have access to people who knew or know the subject (e.g. biography of a family member).

These days, much of the research will likely take place online. It’s crucial, therefore, to provide your students with guidance on how to use the internet safely and evaluate online sources for reliability. This is the era of ‘ fake news ’ and misinformation after all!

BIOGRAPHY WRITING Tip #3: Find Your Themes In Biography Writing

Though predominantly a nonfiction genre, the story still plays a significant role in good biography writing. The skills of characterization and plot structuring are transferable here. And, just like in fiction, exploring themes in a biographical work helps connect the personal to the universal. Of course, these shouldn’t be forced; this will make the work seem contrived, and the reader may lose faith in the truthfulness of the account. A biographer needs to gain and maintain the trust of the reader.

Fortunately, themes shouldn’t need to be forced. A life well-lived is full of meaning, and the themes the student writer is looking for will emerge effortlessly from the actions and events of the subject’s life. It’s just a case of learning how to spot them.

One way to identify the themes in a life is to look for recurring events or situations in a person’s life. These should be apparent from the research completed previously. The students should seek to identify these patterns that emerge in the subject’s life. For example, perhaps they’ve had to overcome various obstacles throughout different periods of their life. In that case, the theme of overcoming adversity is present and has been identified.

Usually, a biography has several themes running throughout, so be sure your students work to identify more than one theme in their subject’s life.

BIOGRAPHY WRITING Tip: #4 Put Something of Yourself into the Writing

While the defining feature of a biography is that it gives an account of a person’s life, students must understand that this is not all a biography does. Relating the facts and details of a subject’s life is not enough. The student biographer should not be afraid to share their thoughts and feelings with the reader throughout their account of their subject’s life.

The student can weave some of their personality into the fabric of the text by providing commentary and opinion as they relate the events of the person’s life and the wider social context at the time. Unlike the detached and objective approach we’d expect to find in a history textbook, in a biography, student-writers should communicate their enthusiasm for their subject in their writing.

This makes for a more intimate experience for the reader, as they get a sense of getting to know the author and the subject they are writing about.

We have compiled a sequence of biography-related lessons or teaching ideas that you can follow as you please. They are straightforward enough for most students to follow without further instruction.

BIOGRAPHY LESSON IDEA # 1:

This session aims to give students a broader understanding of what makes a good biography.

Once your students have compiled a comprehensive checklist of the main features of a biography, allow them to use it to assess some biographies from your school library or on the internet using the feature checklist.

When students have assessed a selection of biographies, take some time as a class to discuss them. You can base the discussion around the following prompts:

  • Which biographies covered all the criteria from their checklist?
  • Which biographies didn’t?
  • Which biography was the most readable in terms of structure?
  • Which biography do you think was the least well-structured? How would you improve this?

Looking at how other writers have interpreted the form will help students internalize the necessary criteria before attempting to produce a biography. Once students have a clear understanding of the main features of the biography, they’re ready to begin work on writing a biography.

When the time does come to put pen to paper, be sure they’re armed with the following top tips to help ensure they’re as well prepared as possible.

BIOGRAPHY LESSON IDEA # 2:

This session aims to guide students through the process of selecting the perfect biography subject.

Instruct students to draw up a shortlist of three potential subjects for the biography they’ll write.

Using the three criteria mentioned in the writing guide (Interest, Merit, and Information), students award each potential subject a mark out of 5 for each of the criteria. In this manner, students can select the most suitable subject for their biography.

BIOGRAPHY LESSON IDEA # 3:

This session aims to get students into the researching phase, then prioritise and organise events chronologically.

Students begin by making a timeline of their subject’s life, starting with their birth and ending with their death or the present day. If the student has yet to make a final decision on the subject of their biography, a family member will often serve well for this exercise as a practice exercise.

Students should research and gather the key events of the person’s life, covering each period of their life from when they were a baby, through childhood and adolescence, right up to adulthood and old age. They should then organize these onto a timeline. Students can include photographs with captions if they have them.

They can present these to the class when they have finished their timelines.

BIOGRAPHY LESSON IDEA # 4:

Instruct students to look over their timeline, notes, and other research. Challenge them to identify three patterns that repeat throughout the subject’s life and sort all the related events and incidents into specific categories.

Students should then label each category with a single word. This is the thematic concept or the broad general underlying idea. After that, students should write a sentence or two expressing what the subject’s life ‘says’ about that concept.

This is known as the thematic statement . With the thematic concepts and thematic statements identified, the student now has some substantial ideas to explore that will help bring more profound meaning and wider resonance to their biography.

BIOGRAPHY LESSON IDEA # 5:

Instruct students to write a short objective account of an event in their own life. They can write about anyone from their past. It needn’t be more than a couple of paragraphs, but the writing should be strictly factual, focusing only on the objective details of what happened.

Once they have completed this, it’s time to rewrite the paragraph, but they should include some opinion and personal commentary this time.

The student here aims to inject some color and personality into their writing, to transform a detached, factual account into a warm, engaging story.

Introductions

What this handout is about.

This handout will explain the functions of introductions, offer strategies for creating effective introductions, and provide some examples of less effective introductions to avoid.

The role of introductions

Introductions and conclusions can be the most difficult parts of papers to write. Usually when you sit down to respond to an assignment, you have at least some sense of what you want to say in the body of your paper. You might have chosen a few examples you want to use or have an idea that will help you answer the main question of your assignment; these sections, therefore, may not be as hard to write. And it’s fine to write them first! But in your final draft, these middle parts of the paper can’t just come out of thin air; they need to be introduced and concluded in a way that makes sense to your reader.

Your introduction and conclusion act as bridges that transport your readers from their own lives into the “place” of your analysis. If your readers pick up your paper about education in the autobiography of Frederick Douglass, for example, they need a transition to help them leave behind the world of Chapel Hill, television, e-mail, and The Daily Tar Heel and to help them temporarily enter the world of nineteenth-century American slavery. By providing an introduction that helps your readers make a transition between their own world and the issues you will be writing about, you give your readers the tools they need to get into your topic and care about what you are saying. Similarly, once you’ve hooked your readers with the introduction and offered evidence to prove your thesis, your conclusion can provide a bridge to help your readers make the transition back to their daily lives. Note that what constitutes a good introduction may vary widely based on the kind of paper you are writing and the academic discipline in which you are writing it. If you are uncertain what kind of introduction is expected, ask your instructor.

Why bother writing a good introduction?

You never get a second chance to make a first impression. The opening paragraph of your paper will provide your readers with their initial impressions of your argument, your writing style, and the overall quality of your work. A vague, disorganized, error-filled, off-the-wall, or boring introduction will probably create a negative impression. On the other hand, a concise, engaging, and well-written introduction will start your readers off thinking highly of you, your analytical skills, your writing, and your paper.

Your introduction is an important road map for the rest of your paper. Your introduction conveys a lot of information to your readers. You can let them know what your topic is, why it is important, and how you plan to proceed with your discussion. In many academic disciplines, your introduction should contain a thesis that will assert your main argument. Your introduction should also give the reader a sense of the kinds of information you will use to make that argument and the general organization of the paragraphs and pages that will follow. After reading your introduction, your readers should not have any major surprises in store when they read the main body of your paper.

Ideally, your introduction will make your readers want to read your paper. The introduction should capture your readers’ interest, making them want to read the rest of your paper. Opening with a compelling story, an interesting question, or a vivid example can get your readers to see why your topic matters and serve as an invitation for them to join you for an engaging intellectual conversation (remember, though, that these strategies may not be suitable for all papers and disciplines).

Strategies for writing an effective introduction

Start by thinking about the question (or questions) you are trying to answer. Your entire essay will be a response to this question, and your introduction is the first step toward that end. Your direct answer to the assigned question will be your thesis, and your thesis will likely be included in your introduction, so it is a good idea to use the question as a jumping off point. Imagine that you are assigned the following question:

Drawing on the Narrative of the Life of Frederick Douglass , discuss the relationship between education and slavery in 19th-century America. Consider the following: How did white control of education reinforce slavery? How did Douglass and other enslaved African Americans view education while they endured slavery? And what role did education play in the acquisition of freedom? Most importantly, consider the degree to which education was or was not a major force for social change with regard to slavery.

You will probably refer back to your assignment extensively as you prepare your complete essay, and the prompt itself can also give you some clues about how to approach the introduction. Notice that it starts with a broad statement and then narrows to focus on specific questions from the book. One strategy might be to use a similar model in your own introduction—start off with a big picture sentence or two and then focus in on the details of your argument about Douglass. Of course, a different approach could also be very successful, but looking at the way the professor set up the question can sometimes give you some ideas for how you might answer it. (See our handout on understanding assignments for additional information on the hidden clues in assignments.)

Decide how general or broad your opening should be. Keep in mind that even a “big picture” opening needs to be clearly related to your topic; an opening sentence that said “Human beings, more than any other creatures on earth, are capable of learning” would be too broad for our sample assignment about slavery and education. If you have ever used Google Maps or similar programs, that experience can provide a helpful way of thinking about how broad your opening should be. Imagine that you’re researching Chapel Hill. If what you want to find out is whether Chapel Hill is at roughly the same latitude as Rome, it might make sense to hit that little “minus” sign on the online map until it has zoomed all the way out and you can see the whole globe. If you’re trying to figure out how to get from Chapel Hill to Wrightsville Beach, it might make more sense to zoom in to the level where you can see most of North Carolina (but not the rest of the world, or even the rest of the United States). And if you are looking for the intersection of Ridge Road and Manning Drive so that you can find the Writing Center’s main office, you may need to zoom all the way in. The question you are asking determines how “broad” your view should be. In the sample assignment above, the questions are probably at the “state” or “city” level of generality. When writing, you need to place your ideas in context—but that context doesn’t generally have to be as big as the whole galaxy!

Try writing your introduction last. You may think that you have to write your introduction first, but that isn’t necessarily true, and it isn’t always the most effective way to craft a good introduction. You may find that you don’t know precisely what you are going to argue at the beginning of the writing process. It is perfectly fine to start out thinking that you want to argue a particular point but wind up arguing something slightly or even dramatically different by the time you’ve written most of the paper. The writing process can be an important way to organize your ideas, think through complicated issues, refine your thoughts, and develop a sophisticated argument. However, an introduction written at the beginning of that discovery process will not necessarily reflect what you wind up with at the end. You will need to revise your paper to make sure that the introduction, all of the evidence, and the conclusion reflect the argument you intend. Sometimes it’s easiest to just write up all of your evidence first and then write the introduction last—that way you can be sure that the introduction will match the body of the paper.

Don’t be afraid to write a tentative introduction first and then change it later. Some people find that they need to write some kind of introduction in order to get the writing process started. That’s fine, but if you are one of those people, be sure to return to your initial introduction later and rewrite if necessary.

Open with something that will draw readers in. Consider these options (remembering that they may not be suitable for all kinds of papers):

  • an intriguing example —for example, Douglass writes about a mistress who initially teaches him but then ceases her instruction as she learns more about slavery.
  • a provocative quotation that is closely related to your argument —for example, Douglass writes that “education and slavery were incompatible with each other.” (Quotes from famous people, inspirational quotes, etc. may not work well for an academic paper; in this example, the quote is from the author himself.)
  • a puzzling scenario —for example, Frederick Douglass says of slaves that “[N]othing has been left undone to cripple their intellects, darken their minds, debase their moral nature, obliterate all traces of their relationship to mankind; and yet how wonderfully they have sustained the mighty load of a most frightful bondage, under which they have been groaning for centuries!” Douglass clearly asserts that slave owners went to great lengths to destroy the mental capacities of slaves, yet his own life story proves that these efforts could be unsuccessful.
  • a vivid and perhaps unexpected anecdote —for example, “Learning about slavery in the American history course at Frederick Douglass High School, students studied the work slaves did, the impact of slavery on their families, and the rules that governed their lives. We didn’t discuss education, however, until one student, Mary, raised her hand and asked, ‘But when did they go to school?’ That modern high school students could not conceive of an American childhood devoid of formal education speaks volumes about the centrality of education to American youth today and also suggests the significance of the deprivation of education in past generations.”
  • a thought-provoking question —for example, given all of the freedoms that were denied enslaved individuals in the American South, why does Frederick Douglass focus his attentions so squarely on education and literacy?

Pay special attention to your first sentence. Start off on the right foot with your readers by making sure that the first sentence actually says something useful and that it does so in an interesting and polished way.

How to evaluate your introduction draft

Ask a friend to read your introduction and then tell you what they expect the paper will discuss, what kinds of evidence the paper will use, and what the tone of the paper will be. If your friend is able to predict the rest of your paper accurately, you probably have a good introduction.

Five kinds of less effective introductions

1. The placeholder introduction. When you don’t have much to say on a given topic, it is easy to create this kind of introduction. Essentially, this kind of weaker introduction contains several sentences that are vague and don’t really say much. They exist just to take up the “introduction space” in your paper. If you had something more effective to say, you would probably say it, but in the meantime this paragraph is just a place holder.

Example: Slavery was one of the greatest tragedies in American history. There were many different aspects of slavery. Each created different kinds of problems for enslaved people.

2. The restated question introduction. Restating the question can sometimes be an effective strategy, but it can be easy to stop at JUST restating the question instead of offering a more specific, interesting introduction to your paper. The professor or teaching assistant wrote your question and will be reading many essays in response to it—they do not need to read a whole paragraph that simply restates the question.

Example: The Narrative of the Life of Frederick Douglass discusses the relationship between education and slavery in 19th century America, showing how white control of education reinforced slavery and how Douglass and other enslaved African Americans viewed education while they endured. Moreover, the book discusses the role that education played in the acquisition of freedom. Education was a major force for social change with regard to slavery.

3. The Webster’s Dictionary introduction. This introduction begins by giving the dictionary definition of one or more of the words in the assigned question. Anyone can look a word up in the dictionary and copy down what Webster says. If you want to open with a discussion of an important term, it may be far more interesting for you (and your reader) if you develop your own definition of the term in the specific context of your class and assignment. You may also be able to use a definition from one of the sources you’ve been reading for class. Also recognize that the dictionary is also not a particularly authoritative work—it doesn’t take into account the context of your course and doesn’t offer particularly detailed information. If you feel that you must seek out an authority, try to find one that is very relevant and specific. Perhaps a quotation from a source reading might prove better? Dictionary introductions are also ineffective simply because they are so overused. Instructors may see a great many papers that begin in this way, greatly decreasing the dramatic impact that any one of those papers will have.

Example: Webster’s dictionary defines slavery as “the state of being a slave,” as “the practice of owning slaves,” and as “a condition of hard work and subjection.”

4. The “dawn of man” introduction. This kind of introduction generally makes broad, sweeping statements about the relevance of this topic since the beginning of time, throughout the world, etc. It is usually very general (similar to the placeholder introduction) and fails to connect to the thesis. It may employ cliches—the phrases “the dawn of man” and “throughout human history” are examples, and it’s hard to imagine a time when starting with one of these would work. Instructors often find them extremely annoying.

Example: Since the dawn of man, slavery has been a problem in human history.

5. The book report introduction. This introduction is what you had to do for your elementary school book reports. It gives the name and author of the book you are writing about, tells what the book is about, and offers other basic facts about the book. You might resort to this sort of introduction when you are trying to fill space because it’s a familiar, comfortable format. It is ineffective because it offers details that your reader probably already knows and that are irrelevant to the thesis.

Example: Frederick Douglass wrote his autobiography, Narrative of the Life of Frederick Douglass, An American Slave , in the 1840s. It was published in 1986 by Penguin Books. In it, he tells the story of his life.

And now for the conclusion…

Writing an effective introduction can be tough. Try playing around with several different options and choose the one that ends up sounding best to you!

First you need to choose a person to write about. The person should have made significant contributions to his/her respective field. This person should also be well-known as you will need resources to find information.

For this guide, we will be writing our biography essay on French-Algerian philosopher and author Albert Camus . There’re some reasons why Albert Camus is a suitable topic for a biography essay:

Gather information on the person

You need some information about your the person you chose. While getting this information, pay attention to two main factors: the credibility of your source and what information you are putting in your essay.

7 Different Ways to Write a Great Biography

Ever considered writing a biography? Individual decisions and circumstances shape life stories, but so do biographers. By adapting set patterns, writers determine public opinion of their subject’s lives. Draw inspiration for a future project from this roundup of common approaches.

Journalists and media outlets love biographies, particularly when relatives or academics dispute the most controversial claims.

Some of the favorite topics are instantly familiar: Napoleon’s downfall, Churchill’s leadership, Diana’s letters and her lovers, Sylvia Plath’s relationship with Ted Hughes, the genius of Steve Jobs, and Alan Turing’s sexuality. Each worthy of separate, in-depth discussion. Each a delicate balance between sensationalism and historical interest.

Of course, the trademark combination of gossip and mythmaking has given biography a bad reputation. For some, it seems too much like rummaging through the paper bin, looking for someone’s bank statements or the shreds of a discarded missive.

Or else it seems like a dubious exercise in trying to draw life lessons from someone else’s fame and success, which might have been coincidental or undeserved.

Laying the Groundwork

Researching a biography involves a lot of borrowing and persuading. Anecdotes, interviews, letters and public records are the standard ingredients of every book biography, film biopic, or feature ‘based on a true story’. Getting hold of information may be difficult.

Relatives of the deceased may block access to the diary, friends of the family may demand cash for answering your questions, and obtaining permission to reproduce images will give you grey hairs. You may be overwhelmed by the quantity of books to plough through, or frustrated by the lack of data at your disposal.

If you’re lucky, you’ll stumble across something no-one else has found before and hope it makes waves. If you’re underhand, you’ll make an unverifiable claim and wait for the public outrage.

But let’s assume that you’ve been principled. You’ve found a worthy subject, done the laborious work of searching through the archives and ringing through the phone book, read the relevant literature and thought about the ethical dilemmas. Now it’s time to write, but where should you begin? How do you bring order to the chaos of a life?

1. Cradle to Grave

If in doubt, the ‘cradle to grave’ approach is your fallback option. Put your notes in order, get the chronology sorted, and start work. One by one, tick off the following from your list: birth, family background, childhood influences, schooling and education, early career, professional successes and setbacks, twilight years, death. Choose a first sentence a bit like this:

“Napoleon Buonaparte was born at Ajaccio in the island of Corsica, on the fifteenth day of August, 1769. He was the son of Charles Buonaparte, an advocate in the royal court of assize, and of Letitia Ramolini, a Corsican lady of great beauty, and of a good family, descended from that of Colalto at Naples.” William Hazlitt, The Life of Napoleon Buonaparte , 1828

As the conventional approach in Western book biographies for hundreds of years, this may sound like the easy option. Yet a chronological biography has its pitfalls. Expect gaps in the story, mysteries you’ll never solve, and conflicting accounts.

Establishing causality is another dilemma and not only because it’s tricky to prove links between particular experiences and later events. Strands of the story developing in parallel, encounters that only obtain significance many years later, and the after-effects of major turning points all pose a challenge to the apparent simplicity of this approach – also see our biographical piece on Emmeline Pankhurst for an example of the pitfalls and opportunities of a ‘cradle to grave’ story.

2. The Deathbed Departure

Like Agatha Christie, many biographers hold off checking the birth certificate by beginning at the end. Opening with a deathbed scene or the public announcement of the death is a ubiquitous variation on the ‘cradle-to-grave’ structure.

Think of the exaggerated public mourning in a Buenos Aires cinema at the beginning of the film adaptation of Evita , followed immediately by sepia-coloured evocations of Eva’s provincial childhood.

By contrasting a dramatic demise with humble beginnings, you can immediately establish both suspense and a narrative arc.

3. Trace Your Steps

If your research process deserves a book of its own, or if your subject was hard to track down, you may want to put the biographical mechanics on display. Documenting the process of biographical research also allows you to write someone else’s story in the first-person. By revealing your techniques and the problems you faced, you can mitigate for the inevitable causal leaps or puzzling gaps.

Literary historians like to cite A. J. A. Symons’s The Quest for Corvo (1934), but it’s a technique found in other genres, such as documentary theatre. For example, Ivna Žic’s play Blei (2017) sees a young woman enlist her friends to reconstruct her grandfather’s experience of the disputed Bleiburg repatriations of 1945, including video interviews, excerpts from books, and taped phone calls.

4. Make It Up

Plenty of would-be historical biographies contain made-up stories impossible to verify, such as the wholly speculative story of Shakespeare’s encounter with Elizabeth I at Kenilworth, enthusiastically mythologized by nineteenth-century biographers.

Given biography’s tendency to improvise with anecdotes and dodgy causal connections, critics say it’s a kind of fiction masquerading as history.

You can make a virtue of a necessity by augmenting the historical sources, as in Edmund Morris’s Dutch: A Memoir of Ronald Reagan (1999). Or else turn the practice of biography on its head by writing it as historical fiction, as in Hilary Mantel’s bestselling Wolf Hall (2009). Familiar representatives of the genre include the films Amadeus (1984) and Shakespeare in Love (1998), both inspired by long-standing myths associated with the lives of Mozart and Shakespeare.

If you aim to popularize a life or just to convey the atmosphere of the times, then so-called ‘biofiction’ allows you to indulge your imagination and free the life story from the strictures of the historical record.

5. Change the Received Wisdom

Myths and legends proliferating? Promise a glimpse behind the scenes and unmask your subject with a revisionist biography. Celebrities’ public personas – historical or contemporary – can easily deceive. By deconstructing appearances and identifying discourses, you can add academic sobriety to fiercely contested terrain, as in Sarah Churchwell’s The Many Lives of Marilyn Monroe (2004).

You can also throw caution to the winds with a thorough debunking. This is the kind of biography that likes to offend. You can do damage to the subject’s reputation (and possibly your own) by focusing on character flaws or allegations of a moral nature. You’ll need persuasive evidence and a biographee long since deceased – that or a good lawyer.

Following the development of ideas across a life, biographies of great minds can play an important role in public understandings of science. You’ll also be looking at how institutions or cultural and historical contexts influenced your subject, how networks champion or resist particular ideas, and how even the best ones are greeted with scepticism.

Challenges include making the material comprehensible for a non-specialist and turning the genesis of complex thoughts into a compelling narrative. Feel free to do something inventive – Darwin’s great-great-granddaughter Ruth Padel wrote a biography of her forebear in poems.

7. A Single Chapter

Very often, society values a given life for a single episode within it. In ‘History as a Poetess’ (1943), Stefan Zweig calls these history’s ‘heroic, poetic moments’.

Reducing a life course to a representative year or two may depart from the genre’s established conventions. But as James Shapiro has demonstrated in two books on Shakespeare, it gives you the chance to focus on what was most important in a life – or at least to make that case.

Collective biographies can do the same for groups. As in Lara Feigel’s The Love Charm of Bombs: Restless Lives in the Second World War (2013), seeing how people’s lives interweave and diverge enables a more personal and unexpected take on familiar historical events.

First impressions are important. Scientists need to make their work stand out among a sea of others. However, many mistakenly believe that first impressions are formed based only on titles and abstracts. In actuality, the introduction section is critical to making a real impression on the audience. The introduction is where authors outline their research topic and describe their study. It is where they provide background information and showcase their writing and argumentation styles. For these reasons, the introduction engages the audience in a deeper way than the formalities and rigidities of the title and abstract can afford. To use a fishing analogy: if the title and the abstract serve as the hook and the bait, then the introduction is the process of actually reeling the fish into the boat.

Good Introductions Are Important Guides

In contrast to the constraints placed on the title and abstract, the introduction is the first real opportunity for the scientist to engage with their audience and showcase and convey their passions and motivations for the study in question. This opportunity is somewhat of a double-edged sword. Study authors inevitably have a treasure trove of knowledge and expertise when it comes to their projects and their fields. However, they must remember that the audience does not necessarily have this background information—and that they are only engaging with their audience for a finite amount of time. Despite the urge to excitedly write about all of the different aspects and intricacies of the project, it is very important that authors keep their introductions simple and well organized. 

Therefore, the introduction should move from broad scopes to narrow focuses as the audience reads further. The author should direct the reader along this journey, focusing on topics with direct relevance to what was investigated in the study. A broad fact introduced early on should be linked or paired with a more specific fact along the same lines of thought, eventually culminating in how this information led to the motivation behind the study itself. It is vital to not go off on tangents or talk about things that are too esoteric. A confused audience is an audience that tends not to read further.

Applying Common Principles Across Well-Known and Niche Subjects

Writers can apply these principles in more specialized manuscripts focusing on a single entity rather than a well-known pathology. Consider the following example from a manuscript by cell biologist Luis R. Cruz-Vera’s research team from the University of Alabama in Huntsville, published in the Journal of Biological Chemistry. 1

Here, they divide the opening paragraph of their introduction into four distinct sections. First, they explain what ribosome arresting peptides (RAPs) are and what they do.

Ribosome arresting peptides (RAPs) are nascent polypeptides that act in cis on the translating ribosome to control the expression of genes by inducing ribosome arrest during translation elongation or termination. RAPs commonly sense external forces or low molecular weight compounds in the environment that spatially and temporally contribute to the expression of genes. 

Then they introduce the two different types of RAPs.

RAPs such as SecM that sense external forces on the ribosome are typically large, because these nascent peptides have a domain that functions outside of the ribosome. In contrast, those that sense small molecules inside of the ribosome, such as TnaC are smaller. 

They describe how each type works via a different mechanism.

Typically, larger RAPs interact with cellular factors that can control their capacity for arresting ribosomes. Because of their size and proximity to ribosomal components, large RAPs clearly show two structural domains, a sensor domain and an arresting domain. At the moment of the arrest for the large RAPs, the sensor domain is located outside the ribosome exit tunnel, whereas the arresting domain remains inside the tunnel. The short RAPs currently characterized interact with the compounds that they sense by using the ribosome exit tunnel as a binding surface. For these short RAPs, it has been determined that conserved amino acid residues are necessary to induce arrest by either directly binding the effector molecule or by acting at the peptidyl-transferase center (PTC) during ribosome arrest. 

And finally, they conclude by highlighting a knowledge gap in how small RAPs operate versus what is already known about large RAPs.

However, because the size of short RAPs ranges from only a few to a couple of dozen amino acids, as in the case of TnaC, it has remained unclear whether short RAPs are constituted by the two independent sensor and stalling domains, as it has been observed with larger RAPs.

In this way, the authors make a natural progression from “why this topic is important” to “what is known about this topic,” setting the stage for “what is unknown about this topic and why it should be studied.” 

Gradually Moving from Broad to Narrow

A three-step funnel explaining how the introduction guides the reader from summary to specific. The first phase should lay out the question that needs to be answered. The second phase should delve deeper into that question, and the final phase should tie what is already known with what is explored in this study.

These principles can be further transferred towards the introductory section as a whole. The first paragraph should serve as an introduction to the field and the topic. The middle paragraph(s) provide exposition and detail regarding what is known and unknown, and what has already been done and still remains to do, and the final paragraph outlines the study and its principle findings, providing a transition into either the materials and methods or the results section. 

For example, this work by radiation oncologist Eric Deutsch’s group at Université Paris-Saclay, published in PLoS One , 2 opens by succinctly explaining a scientific problem: “ the threat of extensive dispersion of radioactive isotopes within populated areas that would have an unfortunate effect on human health has increased drastically .” It then offers the call to action necessitated by this problem: “ the development of a decorporating agent capable of effectively mitigating the effects of a wide range of isotopes is critical .”

In the next two paragraphs, the study authors provide information on how and why dispersion of radioactive isotopes are a problem—“ the FDA has approved only three compounds (only one of which is used as a preventative therapy) for the treatment of exposure to specific radioactive elements ”—and highlights the strengths and weaknesses of what is currently available. They then introduce the focal point of their own work, chitosan@DOTAGA, within this context, explaining its potential as a solution to the problem they previously introduced: “ After oral administration to rodents over several days, no signs of acute or chronic toxicity were observed, and DOTAGA did not enter the blood stream and was fully eliminated from the gastrointestinal tract within 24 hours of administration. ”

Finally, the introduction concludes by listing the study objective—“ explore the potential of this polymer for use in the decorporation of a wide range of radioactive isotopes ”—and the motivations and rationale behind the study objective—“ there are no suitable countermeasures available for uranium poisoning. […] This innovative approach aims to directly chelate the radioactive cations, specifically uranium, within the gastrointestinal tract prior to their systemic absorption, which ensures their prompt elimination and mitigation of the associated toxicities. ”

The Introduction Engages with the Reader

The introduction section is often overlooked in favor of the title and the abstract, but it serves two important functions. First, it gives the audience all of the information that it needs to contextualize the yet-to-be-presented data within the context of the problem that needs to be solved or the scientific question that needs to be addressed. Second, and more importantly, it justifies the importance of the study, of its initiative, rationale, and purpose. The introduction is the author’s best—and arguably only real—opportunity to convince the audience that their study is worth reading.

Differences in end-of-life care patterns between types of hospice used for cancer patients: a retrospective cohort study

In response to the rapid aging population and increasing number of cancer patients, discussions on dignified end-of-life (EoL) decisions are active around the world. Therefore, this study aimed to identify the differences in EoL care patterns between types of hospice used for cancer patients.

In this population-based cohort study, the Korean National Health Insurance Service cohort data containing all registered cancer patients who died between 2017 and 2021 were used. A total of 408,964 individuals were eligible for analysis. The variable of interest, the type of hospice used in the 6 months before death, was classified as follows: (1) Non-hospice users; (2) Hospital-based hospice single users; (3) Home-based hospice single users; (4) Combined hospice users. The outcomes were set as patterns of care, including intense care and supportive care. To identify differences in care patterns between hospice types, a generalized linear model with zero-inflated negative binomial distribution was applied.

Hospice enrollment was associated with less intense care and more supportive care near death. Notably, those who used combined hospice care had the lowest probability and frequency of receiving intense care (aOR: 0.18, 95% CI: 0.17–0.19, aRR: 0.47, 95% CI: 0.44–0.49), while home-based hospice single users had the highest probability and frequency of receiving supportive care (Prescription for narcotic analgesics, aOR: 2.95, 95% CI: 2.69–3.23, aRR: 1.45, 95% CI: 1.41–1.49; Mental health care, aOR: 3.40, 95% CI: 3.13–3.69, aRR: 1.35, 95% CI: 1.31–1.39).

Our findings suggest that although intense care for life-sustaining decreases with hospice enrollment, QoL at the EoL actually improves with appropriate supportive care. This study is meaningful in that it not only offers valuable insight into hospice care for terminally ill patients, but also provides policy implications for the introduction of patient-centered community-based hospice services.

Introduction

In recent decades, cancer has emerged as the primary cause of mortality worldwide. In 2020, approximately 10 million people had died from cancer, accounting for roughly one-sixth of all recorded deaths [ 1 ]. Although notable improvements in cancer detection and treatment have extended the lifespans of many cancer patients, many patients continue to be diagnosed at advanced terminal stages. Moreover, patients requiring advanced care often contend with physical and psychological symptoms stemming from their illnesses, treatment, or concurrent health issues [ 2 ]. Regrettably, these symptoms are frequently left unaddressed by conventional medical care, thereby affecting the patients’ well-being and relationships with their families [ 3 , 4 , 5 ]. Accordingly, hospice and palliative care initiatives were introduced to enhance the quality of life (QoL) of terminally ill patients and their caregivers by prioritizing relief rather than cure [ 6 ].

Hospice and palliative care are integral components of patient-centered healthcare and a part of a global ethical obligation to mitigate profound impacts of severe health conditions, encompassing physical, emotional, and spiritual dimensions [ 7 ]. The World Health Organization estimates that approximately 56.8 million individuals, including 25.7 million in their final year of life, require palliative care annually [ 8 , 9 ]. This demand is rising owing to global aging trends and the increasing prevalence of chronic illnesses such as cancer, heart disease, and dementia. However, the current provision of palliative care falls far short of meeting this need, reaching only approximately 14% of people requiring the services [ 8 , 9 ].

Furthermore, in response to Korea’s rapidly aging population, deliberations regarding dignified end-of-life (EoL) decisions are ongoing. In 2016, the Korean National Assembly enacted the “Act on Hospice and Palliative Care and Decision on Life-Sustaining Treatment for Patients at End of Life” [ 10 , 11 ], which permits terminally ill patients to make the choice to forego life-sustaining treatment (LST). In Korea, cancer accounts for one in every four deaths, and approximately 23.2% of all patients with cancer-related deaths are involved in hospice care services [ 12 ], indicating LST withdrawal. Although three types of hospice services—hospital-based, home-based, and consultative hospices—have been introduced in Korea, most patients opt for hospital-based hospice care. Only 4% of patients choose home-based hospice care [ 12 ], whereas consultative hospice services serve as a supplementary step before patient enrollment in hospital- or home-based hospice care. This current state of hospice utilization in Korea prompted us to consider the efficacy of this policy.

Therefore, in this study, we aimed to identify differences in EoL care patterns among the types of hospices used for cancer patients. We believe that our findings can provide information on the use of hospice care as a means for terminally ill patients to exercise their right to self-determination at the end of their lives.

Data and study population

In this population-based cohort study, we obtained data from the Korean National Health Insurance Service (NHIS) database. Since the implementation of universal health coverage in 1989, all South Korean citizens have been obliged to subscribe to the NHIS, and approximately 98% of the entire population has been enrolled. The NHIS database includes the International Classification of Diseases 10th revision (ICD-10) diagnostic codes, prescriptions for medications, length of hospital stay, medical expenses, and information regarding healthcare provisions [ 13 ].

To explore the impact of the type of hospice used on cancer patients’ EoL care patterns, we included cancer patients who died after registering for expanded benefit coverage due to severe cancer (claim code: “V193”). Subsequently, we obtained the NHIS cohort data of all 521,452 registered cancer patients who died between January 1, 2017 and December 31, 2021. We excluded those who survived > 5 years after their first cancer diagnosis, had no medical records for 6 months before death, and aged < 20 years or had missing data were sequentially excluded, resulting in a total of 477,203 participants. Among them, 76,894 individuals had used hospice care within 6 months before death, whereas 400,309 did not. Because we aimed to investigate care patterns and expenditures 1 and 3 months before death based on hospice care use, we excluded patients who died within 3 months of their cancer diagnosis. As determining patients who died on the day of hospice enrollment in the intervention group, we also excluded such patients ( n  = 956). Finally, 408,964 individuals were eligible for analysis, of which 67,522 and 341,422 individuals were in the intervention and control groups, respectively. A flowchart of the study sample selection process is shown in Fig.  1 .

The dependent variables were the patterns of care, which were divided into intense and supportive care. Intense care refers to aggressive treatment to prolong the life of cancer patients [ 14 ]; in this study, it was specifically defined as intubation and ventilator use, cardiopulmonary resuscitation, hemodialysis, care in the intensive care unit (ICU), or computed tomography (CT) use. Supportive care refers to pain control and psychological relief management that can significantly impact the cancer patients’ QoL [ 15 , 16 , 17 , 18 ] and was defined as a prescription of narcotic analgesics and visits to psychiatry and family medicine clinics (Supplementary Table 1 ). For these outcomes, whether the patient received care in the last 30 and 90 days of life was identified as a binary variable, and the total amount of care was identified as a count variable.

The type of hospice used in the 6 months before death was classified into four categories as follows: those who have (1) never used hospice (reference group); (2) only used hospital-based hospice (claim codes: “WJ-,” “WK-,” “WL-,” “WM-,” “WN-,” “WO-,” “WG-,” “WH-”); (3) only used home-based hospice (claim codes: “AP-”); and (4) used both hospital- and home-based hospice within 6 months before death.

We included 10 variables in the analysis as covariates. First, as sociodemographic factors, sex (males and females), age (range: < 30, 30–39, 40–49, 50–59, 60–69, and ≥ 70 years), region (Seoul and metropolitan cities, small cities, and rural area), income level (low [quintile, 1–6], middle [7–13], high [14–20]) and type of health insurance subscription (regionally-insured, workplace-insured, and Medicaid) were included in the analysis. Second, as factors related health status, we adjusted for the Charlson comorbidity index (CCI) score (range: 0–1 and ≥ 2), primary cancer type (lung, liver, colorectal, gastric, pancreatic, gallbladder/bile duct, breast, and prostate cancer, non-Hodgkin's lymphoma, leukemia, and other types), survival time after cancer diagnosis (90–365 days, 366–730 days, 731–1,095 days, and ≥ 1,096 days), and the year of death.

Statistical analysis

We used the chi-squared test to examine the distribution of general characteristics of the study population in the year of death. General characteristics are presented as frequencies (n) and percentages (%), whereas descriptive statistics for all dependent variables are reported as means and standard deviations. To identify differences in care patterns between hospice types, a generalized linear model with a zero-inflated negative binomial (ZINB) distribution was applied. Count data containing a large number of zeros are commonly observed across various fields, such as medicine and public health [ 19 , 20 ]. Zero inflation, which often signifies overdispersion, indicates that the frequency of zero counts exceeds expectations. When the overdispersion in raw data is due to zero inflation, the zero-inflated Poisson (ZIP) model serves as a standard framework for fitting the data [ 21 ]. After factoring in zero inflation and if the data persistently indicate further overdispersion, the ZINB model should be considered [ 22 ]. This model combines a distribution degenerate at zero with a baseline negative binomial distribution as an alternative to the ZIP model [ 23 , 24 ]. As a result, the ZINB model had two components [ 25 , 26 ]: First, we estimated the odds ratios (OR) from a logistic regression model (zero component). Second, we estimated the risk ratio (RR) using the results of the negative binomial regression model (count component). In this study, the zero component was modeled to estimate the probability that an excess zero will not occur, that is, a non-zero probability.

All statistical analyses were conducted using the SAS version 9.4 software (SAS Institute Inc., Cary, NC, USA). Statistical significance was set at p  < 0.05.

Table 1 presents the general characteristics of the study population in their year of death. Among all participants, the proportion of patients who were male, aged > 60 years, had high-income, and regionally insured was reported to be high regardless of the type of hospice used. Meanwhile, in the non-hospice user group, more patients were living in small cities or rural areas; however, among home-based hospice only and combined hospice users, many were living in Seoul and metropolitan cities. In addition, among all hospice types, patients with a CCI score of 0–1 and with a survival period > 3 years (1,096 days) were the most frequently reported. Lung and colorectal cancer accounted for the highest proportion of the top 10 primary cancer types. The number of cancer deaths tended to increase every year; however, during the coronavirus disease pandemic in 2020 and 2021, the proportion of patients using only hospital-based and combined hospice services decreased, whereas that of patients who used only home-based hospice and non-hospice users noticeably increased.

Descriptive statistics for intense care are presented in Supplementary Table 2 , and results of the ZINB regression model exploring the differences in intense care according to the type of hospice used are presented in Table  2 . The odds of receiving intense care in the last 30 days of life was significantly lower among hospice users than among non-hospice users. For combined hospice users, the odds was estimated to be as low as 82% (hospital-based hospice only, adjusted OR [aOR]: 0.36, 95% CI: 0.35–0.37; home-based hospice only, aOR: 0.37, 95% CI: 0.34–0.40; combined hospice, aOR: 0.18, 95% CI: 0.17–0.19). Furthermore, analysis using the count model of patients receiving intense care at least once in the last 30 days of life revealed that the number of patients receiving intense care among hospice users was significantly lower than that among non-hospice users. Similar to the results of the logistic model, the difference in the number of patients receiving intense care with that in non-hospice users was the largest in combined hospice users (hospital-based hospice only, adjusted RR [aRR]: 0.57, 95% CI: 0.56–0.58; home-based hospice only, aRR: 0.61, 95% CI: 0.58–0.65; combined hospice, aRR: 0.47, 95% CI: 0.44–0.49). When the outcome was set as intense care during the last 90 days of life, similar results was observed (Supplementary Table  3 ). The probability of receiving intense care in the last 90 days of life was significantly lower among hospice users, irrespective of hospice type, than among non-hospice users. Significant differences were observed in the total number of ICU visits in the last 90 days, depending on the type of hospice used.

Descriptive statistics for supportive care are presented in Supplementary Table  4 , and results of the ZINB regression model exploring the differences in supportive care according to the type of hospice used are presented in Table  3 . The odds of receiving a prescription for narcotic analgesics in the last 30 days of life was significantly higher among hospice users than among nonhospice users. Notably, home-based hospice single users had a 2.95 times higher probability of receiving the prescription than non-hospice users (hospital-based hospice only, aOR: 1.19, 95% CI: 1.15–1.22; home-based hospice only, aOR: 2.95, 95% CI: 2.69–3.23; combined hospice, aOR: 1.98, 95% CI: 1.85–2.13). In addition, analysis using a count model of individuals prescribed narcotic analgesics at least one in their last 30 days of life, the total number of prescriptions in hospice users was significantly higher than that in the non-hospice users (hospital-based hospice only, aRR: 1.39, 95% CI: 1.38–1.41; home-based hospice only, aRR: 1.45, 95% CI: 1.41–1.49; combined hospice, aRR: 1.45, 95% CI: 1.42–1.49). Regarding prescriptions for narcotic analgesics in the last 90 days of life, a similar pattern was observed (Supplementary Table  5 ). A significant difference was observed among the types of hospice used in both the probability of being prescribed narcotic analgesics and total number of prescriptions, with user of only home-based hospice having the highest probability.

Subsequently, we analyzed differences in mental health care before death according to the type of hospice used (Table  4 ). Compared with non-hospice users, all three types of hospice users had approximately four times higher odds of receiving mental health care in their last 30 days of life (hospital-based hospice only, aOR: 3.58, 95% CI: 3.51–3.66; home-based hospice only, aOR: 4.96, 95% CI: 4.58–5.36; combined hospice, aOR: 4.46, 95% CI: 4.22–4.72). The number of mental health care was also significantly associated with the type of hospice used (hospital-based hospice only, aRR: 1.24, 95% CI: 1.22–1.26; home-based hospice only, aRR: 5.91, 95% CI: 5.56–6.29; combined hospice, aRR: 4.91, 95% CI: 4.69–5.15). Particularly, home-based hospice only users were estimated to have received 5.91 times more mental health care in the last 30 days of life than non-hospice users. Similar results from the logistic model were observed when differences in mental health care in the last 90 days of life were set as outcomes (Supplementary Table  6 ). Meanwhile, based on the count model, combined hospice users were estimated to have the largest difference in the total number of mental healthcare services in their last 90 days of life compared with non-hospice users.

In this retrospective cohort study, we examined the differences in care patterns during the last 30 and 90 days of life according to the type of hospice care used. The key findings of this cohort study are summarized as follows: First, hospice use was associated with less intense and more supportive care near death. Notably, combined hospice care users had the lowest probability and frequency of receiving intense care, whereas patients using only home-based hospice had the highest probability and frequency of receiving supportive care. This finding is consistent with those of previous studies reporting that hospice and palliative care are effective in reducing the procedural burden and aggressive care at the EoL [ 27 , 28 ]. Second, we measured narcotic analgesic prescriptions for pain control and psychiatric consultations for psychological relief as supportive care [ 29 , 30 ]. As a result, we found that patients who used only home-based hospice care received superior pain and mental health management, predicting that this would improve their QoL during their final days. This has implications similar to those of US studies, which demonstrated that nursing home residents enrolled in hospice care had better pain management than those not enrolled in hospice [ 31 , 32 ].

Several studies have examined the impact of hospice care on healthcare utilization and costs for terminally ill cancer patients. They suggested that offering hospice care at an earlier stage may reduce unnecessary hospital admissions and healthcare resource utilization [ 33 ]. Furthermore, the adoption of hospice care tends to lower medical expenses by preventing unnecessary medical interventions [ 34 ]. Hospice care also can effectively manage severe pain and enhances the patients’ overall QoL [ 35 , 36 ]. In countries with diverse hospice service offers, researchers have explored the outcomes of hospital- and home-based hospice models. Patients who opt for home-based hospice care receive palliative support at their own residences and eventually pass away in a familiar and comfortable environment. Therefore, insurance mandates for home-based hospice care in Korea were recently introduced. However, this mandate leads to limited number of studies assessing the effectiveness of each type of care; and no studies have evaluated whether this policy has been implemented as intended. Moreover, although a significant number of patients use more than one type of hospice care depending on their health status or preferences, the effects of this multiple use on healthcare utilization and health outcomes have never been evaluated.

Although the domestic hospice use rate has increased compared with that in the past, the rate remains only 23.7% as of 2022 [ 37 ], which is low compared with that of major countries. Even if patients express their intention to withdraw LST and wish to die in their own home, most receive hospice care in a hospital setting at the EoL because of family recommendations or anxiety about their health conditions. This phenomenon is thought to have occurred because awareness of appropriate EoL care, including hospice care, has not yet been properly established in South Korea. Accordingly, the results of this study can provide valuable information and insights for individuals who are hesitant to use hospice care, thereby enabling them to recognize their autonomy rights. Our findings have important implications for the active promotion and development of established policies. Currently, Korea's hospice-eligible diseases include five diseases, including cancer, which is limited compared with those of other major countries. Considering the continuously increasing mortality rates attributable to chronic and geriatric diseases such as dementia, hospice-eligible diseases should be expanded to ensure a dignified EoL for all patients. In addition, based on the policy trends in major countries, Korea should enhance its efforts in advocating for patient-centered community-based hospice care policies. This can be achieved by identifying places where patients express their preference for EoL care or where they would like to spend their final moments.

This study had certain limitations. First, we obtained NHIS cohort data that only included patients who died between 2017 and 2021 after registering for expanded benefit coverage owing to severe cancer; therefore, we were unable to identify medical utilization records, sociodemographic information, and mortality for patients with diseases other than cancer. Hence, individuals who died from hospice-ineligible diseases were not included in the comparison group. Instead, we included cancer patients who died without using hospice care within the same period as the intervention group as a comparison group. Second, although cancer staging is a very important confounder in evaluating outcomes at the EoL of cancer patients, this information was not included in the data we analyzed. Thus, we had to first select people who died after being diagnosed with cancer and then follow them retrospectively. Third, we attempted to measure the duration of hospice use, yet encountered constraints with the data. While the date of hospice enrollment was recorded, the discharge date was absent. Consequently, we couldn’t ascertain whether patients transitioned to a general ward following hospice enrollment or determine the precise duration of hospice use. Fourth, the NHIS cohort dataset was constructed for administrative purposes; therefore, the ICD-10 codes recorded for health insurance claims may not provide detailed clinical information about the patients’ conditions. Furthermore, potential incomplete coding, which could lead to misclassification or underestimation of the outcomes, remains a concern [ 38 , 39 , 40 ]. Finally, we attempted to account for potential factors that could affect EoL care patterns and expenditures in cancer patients, such as primary cancer type, survival time after initial cancer diagnosis, and comorbidities. However, it is important to note that we could not completely eliminate the possible impact of unmeasured variables, which could affect these confounding factors.

Conclusions

Our findings demonstrated that hospice use is associated with receiving less intense and supportive care at the EoL. Notably, because home-based hospice only users receive better pain management and mental health care, their QoL during the final days is expected to improve. Thus, although intense life-sustaining care decreases with hospice enrollment, QoL at the EoL improves with appropriate supportive care. Based on the policy trends of countries with advanced hospice care, developing patient-centered, community-based hospice care policies is advisable. This policy would offer advantages to both the government, by enabling efficient management of medical resources, and patients, who can assert their autonomy and die with dignity and without suffering.
