Federal regulations, licenses, and permits associated with starting your business include doing business as (DBA), health licenses and permits from the Occupational Safety and Health Administration ( OSHA ), trademarks, copyrights, patents, and other intellectual properties, as well as industry-specific licenses and permits. 

You may also need state-level and local county or city-based licenses and permits. The license requirements and how to obtain them vary, so check the websites of your state, city, and county governments or contact the appropriate person to learn more.

As a mitigation strategy, we will diversify our supplier base and keep adequate but not excessive inventory on hand. Continuous training and development will be provided to our staff to ensure compliance with healthcare regulations. To address staffing issues, we build a pipeline of qualified candidates and maintain relations with temporary staffing agencies for prompt fulfillment of workforce needs.

Financial Risks:

Interest rate fluctuations and economic downturns pose financial risks. Such economic changes can affect borrowing costs for any debt-financed aspects of our operations or impact customers' purchasing power, respectively.

Insurance Risks:

Insurance risks pertain to the adequacy and coverage of our insurance policies concerning liability, property, and cyber threats. A lapse in insurance coverage or insufficient coverage can result in devastating financial losses.

NEW YORK (AP) — Walmart is offering new perks for its hourly U.S. workers, ranging from a new bonus plan to opportunities to move into skilled trade jobs within the company.

The perks program announced Wednesday comes as the nation’s largest private employer says it’s seeing a decline in worker turnover. But Walmart, like other employers, faces a still-competitive labor market and increasing demands from its employees .

Walmart’s new bonus plan for eligible part-time and full-time U.S. store workers — including those in its pharmacy and Vision Centers — is based on their store’s performance, with the maximum bonus potential also tied to years of experience.

For example, a full-time worker who’s been with Walmart between one year and almost five years can earn a maximum bonus of $350 per year, while a 20-year full-time worker can earn a maximum bonus of $1,000, Walmart said. The plan will be available to 700,000 U.S. workers, the company said.

In January, Walmart announced its U.S. store managers would receive up to $20,000 in Walmart stock grants every year.

The company, based in Bentonville, Arkansas, is also launching a training program for hourly workers in its U.S. stores and supply networks that will give them an opportunity to move into roles in facilities maintenance, refrigeration, heating, ventilation and air conditioning, and automation. Walmart said it is looking to increase these skilled trades workers from 450 to roughly 2,000 in the next two years.

The jobs pay between $19 and $45 per hour, and workers will be paid during the training, the company said. Walmart’s average hourly wage is close to $18, an increase of 30% over the past five years. Walmart’s starting wages for U.S. workers range between $14 and $19 an hour.

The training program is expected to start with 100 workers in the Dallas-Fort Worth area. It is part of the Walmart Academy, one of the largest skill-development programs in the country.

Last month, Walmart reported another quarter of strong results as its low prices pull in shoppers scouring for discounts amid stubbornly high inflation .

Households with incomes exceeding $100,000 have been drawn to the retailer as it upgrades the quality of its items and focuses on providing more convenient ways to shop. Two-thirds of Walmart’s market share gains across the aisles are coming from that group, Walmart said.

Walmart said the skilled trades initiative is similar to a program it announced two years ago that gave employees who work in its distribution and fulfillment centers a chance to become certified Walmart truck drivers through a 12-week program taught by the company’s established drivers.

At the time, it said it was raising pay for its 12,000 truck drivers. The starting range for new drivers is now between $95,000 and $110,000 a year. The retailer said that $87,500 had been the average that new truck drivers could make in their first year.

Walmart spokeswoman Anne Hatfield said the trucker program has produced more than 500 new drivers since launching in the spring of 2022. That’s helped the company navigate an industrywide shortage of truck drivers.

Walmart has similar training and development programs for pharmacy tech and opticians.

Meanwhile, Walmart said Wednesday it is expanded the number of skills certificates available to help fast-track front-line workers into about 100,000 jobs that are higher paying and in demand at the company over the next three years.

In 2020, the company offered five skilled certificates. Now, there are 50. They include topics such as front-line manager leadership, people and business leadership, data science, software development and project management.

Employees can earn the certificates in four months, according to Lo Stomski, Walmart’s senior vice president and chief talent officer.

Walmart highlighted its worker training efforts as it prepares to hold two big events this week. A virtual shareholders’ meeting is set for Wednesday. On Friday, the company is holding a celebration of its workers at Bud Walton Arena in Fayetteville, Arkansas, to recognize Walmart’s employees from around the world as well as shareholders.

Meeting changing consumer needs: The US retail pharmacy of the future

The US retail pharmacy landscape faces saturated retail locations, ongoing labor shortages, inflationary pressure, and a leveling-off of generic drug penetration. To effectively compete in this environment, all types of retail pharmacies can strive to better understand and adapt to changing consumer preferences. McKinsey conducted a consumer survey of more than 1,000 people in the United States to gain insight into these preferences. 1 McKinsey surveyed 1,004 US adults from November 19 to December 12, 2021, to better understand consumer preferences and expectations of retail pharmacies; these preferences are expected to be relatively consistent in 2022 and 2023. This article lays out how the retail pharmacy landscape has changed over the years, how consumer preferences have evolved, and how different types of retail pharmacies may respond as a result.

Over the past two decades, the retail pharmacy landscape—defined as pharmacies that dispense prescription medication—has evolved substantially. We break down the sector into four types of retail pharmacies: retail chains, regional pharmacies such as mass retail and grocers, independent pharmacies, and mail-order and online pharmacies (Exhibit 1).

Retail chains are the largest and most prevalent of the four pharmacy types, representing a third of stores and about a third of prescription revenues in 2021. On a per-store basis, retail chains dispense approximately 138,000 prescriptions annually—about 50 percent more than grocers, the next largest prescription dispensers per retail location. 2 Adam J. Industry consolidation and gains in efficiency through economies of scale help account for this difference. For example, from 2010 to 2021, CVS and Walgreens acquired a total of nearly 5,000 pharmacy locations, including CVS acquisitions of 1,700 Target pharmacies and Walgreens acquisitions of 1,900 Rite Aid pharmacies. 3 Adam J. Note: Mail order is not included in retail chain prescription revenue figures although some chains own mail-order facilities; these estimates account for the number of days of medication supply. However, the consolidation wave may have already peaked; these organizations are now shifting their focus to improving operating margins. To that end, the companies have announced plans to reduce their footprint. For example, in 2019, Walgreens said it would close 200 US stores; in late 2021, CVS announced that it would “reduce store density in certain locations” and close 900 stores by 2024. 4 Michael Dabaie and Sharon Terlep, “CVS to close 900 stores over three years,” Wall Street Journal , November 18, 2021. As part of this strategic shift, national chains have also begun to explore growth opportunities in adjacencies such as healthcare services, primary care, and vaccinations.

The evolution of national chains has, in turn, put pressure on other retail pharmacy types such as regional pharmacies. This group can be further segmented into two types: grocers (supermarkets that also have a pharmacy) and mass retail (large consumer goods retailers that also have a pharmacy). In 2021, regional pharmacies represented around 30 percent of stores and 15 percent of prescription revenue. 5 Total prescription revenues include specialty revenues; The 2022 economic report , March 2022. On a per-store basis, mass retail and grocers fill 91,000 prescriptions per pharmacy per year on average.

Grocers have experienced variable performance over the past few years. Larger grocers have grown through acquisition, and smaller ones have increasingly been acquired by national chains or larger grocers. For mass retail pharmacies, economies of scale based on brand name and size have enabled them to negotiate incentive-based networks and narrow networks with health plans, pharmacy benefit managers (PBMs), and independent employers, increasing traffic to their stores. 6 Adam J. In contrast to the growth in national chains, the number of independent pharmacies has decreased by nearly 50 percent since 1980, leveling off at about 20,000 locations since 2000. 7 The 2022 economic report , March 2022; “NCPA releases 2021 digest report,” National Community Pharmacists Association (NCPA), October 11, 2021. Independent pharmacies fill about 48,000 prescriptions per pharmacy per year. 8 The 2022 economic report , March 2022. Those that remain have stayed viable through effective collaboration with other independents and wholesalers. This cooperation has taken the form of administrative, operational, and other business management support from wholesalers, pharmacy service administrative organizations, and group-purchasing organizations.

For the fourth type of retail pharmacy, mail-order and online pharmacies, overall penetration for nonspecialty drugs is low compared with penetration for specialty drugs. In 2021, these organizations accounted for less than 10 percent of total US prescriptions, 9 However, because a substantial portion of the prescriptions are specialty, mail-order and online pharmacies represent 37 percent of overall prescription revenues. but they are gaining traction. Historically, mail-order pharmacies were defined as PBM-owned processing facilities that filled maintenance medications centrally. In the past 15 years, a number of direct-to-consumer online pharmacies unaffiliated with PBMs have been established, with this segment receiving more than $3 billion in funding to date. 10 The 2022 economic report , March 2022. In parallel, traditional players such as retail chains have also moved to more omnichannel—physical, online, and mail—options. For example, Walgreens has its own mail-order pharmacy, offers same-day delivery from 8,000 stores, and provides prescription fulfillment through its phone app. 11 “Walgreens launches nationwide rollout of same-day delivery with Instacart,” Walgreens, February 23, 2021.

Shifting consumer preferences will shape the next era of growth

In this evolving environment, retail pharmacies can work to ensure they understand and meet consumers’ needs so they are a preferred destination. The McKinsey survey of more than 1,000 US consumers can help us understand current consumer needs and what may be changing. Four key insights emerged (Table 1). 12 These insights are generalized, and we recognize they may differ when cut by different segments, such as geographic location, number of prescriptions, and insurance.

In this evolving environment, retail pharmacies can work to ensure they understand and meet consumers’ needs so they are a preferred destination.

1. Current pharmacies have high consumer satisfaction

More than two-thirds of survey respondents report satisfaction with their primary retail pharmacy and would recommend it to a friend or colleague. Respondents selected insurance coverage and proximity to home as the top two criteria when selecting a retail pharmacy (Exhibit 2). At the same time, nearly a third of respondents have switched their preferred retail pharmacy in the past five years for one of two reasons: changing insurance coverage or a changing place of residence.

2. Retail chains remain most common primary pharmacy type

According to the survey, retail chains remain the most popular type of primary retail pharmacy, with 47 percent of respondents selecting it. However, the likelihood of respondents reporting a retail chain as their primary pharmacy decreased as the number of monthly medications increased (Exhibit 3).

3. Use of alternative channels is small, but awareness is growing

Despite longstanding PBM mail-order, local-delivery, and (more recently) online options, these alternatives to visiting a brick-and-mortar pharmacy to pick up prescriptions still have relatively low market penetration. Forty-five percent of respondents have never used a mail-order or online pharmacy, and only 13 percent consider a mail-order or online pharmacy to be their primary pharmacy. However, the survey results suggest growing awareness and acceptance of these alternative channels. Forty-four percent of consumers who use a mail-order or home delivery service reported choosing these services over other delivery options more often than they did two or three years ago. Among respondents who are not currently using these services but are willing to consider switching, convenience and price are reported as the top motivating factors.

4. Consumers welcome the expanding role of retail pharmacies

Consumers of all backgrounds appear to welcome all types of retail pharmacies taking on a broader role that encompasses more than prescription dispensing, a trend that is reflected in their spending patterns. At their primary retail pharmacies, consumers reported purchasing a variety of non-prescription-medication products beyond over-the-counter products (48 percent), including food and grocery (36 percent), beauty products (32 percent), and household items (30 percent). Consumers also said they were interested in accessing a broader range of healthcare service offerings in their retail pharmacies, with about four in ten expressing interest in common-illness treatment, whole-health fusion, 13 “Whole-health fusion” refers to services in which pharmacists who are educated in both traditional medicine and alternative remedies are available in stores to advise customers. and other health services, including primary care, dental, lab work, and X-rays.

The implications of evolving consumer preferences

At its most basic level, drug dispensing—filling a prescription accurately and on time—could be considered a commodity service. But given evolving consumer preferences, each type of pharmacy—retail chains, regional pharmacies such as mass retail and grocers, independent pharmacies, and mail-order and online pharmacies—can seek to meet these needs through different methods. Long-term success can be achieved in one of two ways: creating a differentiated experience for the consumer or offering a differentiated set of products and services. This section explores some key questions and opportunities tailored to the unique strengths of each type of retail pharmacy (Table 2).

Retail chains

Retail chains can consider creating more-personalized experiences to attract high-touch consumers with whom independent pharmacies have traditionally held an advantage. Additionally, retail chains can consider new ways to use their scale to offer a differentiated set of services. Two opportunities—omnichannel engagement and healthcare services—show particular promise.

First, as behavior changes sparked by COVID-19 continue beyond the pandemic, consumers will continue to demand digital and omnichannel solutions that allow them to receive pharmacy care when and where they need it most (for example, home delivery, same-day delivery, and on-demand pharmacist support). Retail chains, given their geographical reach and ability to invest in these capabilities, are well positioned to satisfy this demand.

Second, retail chains can best respond to consumer interest in accessing a broad set of healthcare services in the retail pharmacy setting. Retail chains have the balance sheet to build in-house capabilities (for example, to provide primary care and treat common illnesses) or forge at-scale partnerships with external vendors to provide those services.

Regional pharmacies

Regional pharmacies such as mass retail and grocers can consider ways to differentiate themselves based on another factor: integration into daily life. Because they are already embedded into the flow of consumers’ daily lives, these pharmacies can seek to satisfy consumer wellness needs (for example, for healthy foods and over-the-counter supplements) in an integrated way with pharmacy services.

More specifically, mass retail and grocers could be well positioned to become a one-stop shop for all things related to health, creating front- and back-of-store synergies for certain treatment categories. For example, a diabetic consumer may need a full spectrum of products, including prescription drugs, a tailored diet, glucose meters, glucose test strips, eye vitamins, dry-mouth treatments, and skin care products. The mass retail or grocer that can seamlessly fulfill these needs in one location could deliver outsize value to the consumer.

Independent pharmacies

In contrast to the national chains and regional pharmacies, independent pharmacies can bolster their position by further personalizing care, especially for high-touch consumers. They can consider, for example, approaches to satisfy the particular needs of patients with complex chronic conditions or deepen the relationship between pharmacist and patient with personalized services.

While their smaller scale may have traditionally been seen as a challenge, independent pharmacies can also consider satisfying unmet needs in rural or other areas where there is lower penetration by national chains.

Mail-order and online pharmacies

Online pharmacies seeking to disrupt the market can pursue strategies that reinforce their natural benefits (speed and convenience) with their corresponding target demographic (convenience seekers). But they can also work to bridge some of the potential gaps in consumer experience caused by their lower-touch approach. Mail-order and online pharmacies may consider ways to create a more personalized experience that preserves some of the benefits of an in-person experience. For example, video telehealth visits with remote pharmacists could help establish a personal connection to complement the benefits of convenience in the absence of face-to-face consultations.

As the retail pharmacy landscape evolves, different types of pharmacies must seek new ways to meet consumer needs to remain differentiated and relevant. While certain overall trends, including the demand for convenience and the benefits of personalization, are relevant to all types of retail pharmacies, the specific strategy to become a “pharmacy of the future” will vary for each type. There is no one-size-fits-all approach.

Minnesota ag pushing for u.s. supreme court review on pharmacy benefit manager rules.

Minnesota Attorney General Keith Ellison is leading a bipartisan group of 32 attorneys general asking the U.S. Supreme Court to review a case that could otherwise limit state regulation of pharmacy benefit managers (PBMs).

These companies are hired by health insurers to manage drug benefits within heath plans, which includes everything from setting co-pays for medications to creating pharmacy networks where patients can fill prescriptions.

Minnesota, Oklahoma and other states say PBM practices, in some cases, have enriched insurance companies, hurt independent pharmacies and made it harder for patients to afford and access their medicines.

In August 2023, an appellate court sided with the Pharmaceutical Care Management Association, a trade group for PBMs, in a legal challenge to certain regulations for how companies can manage pharmacy networks in Oklahoma.

"Review by this court is therefore necessary to ensure that states continue to be able to enforce needed consumer protections and prevent the blatant abuses that PBMs would otherwise engage in," the attorneys general said in an amicus brief filed Monday with the Supreme Court.

The Pharmaceutical Care Management Association said it would continue to defend the appellate court's decision.

"The Oklahoma law is designed to regulate how employers and unions structure their prescription drug benefits," Greg Lopes, a spokesman for the trade group, said in a statement to the Star Tribune. "Requiring plan sponsors to include unsafe or inefficient pharmacies in their provider networks, and forbidding health plans from using common cost-containment tools like preferred networks, will increase prescription drug costs for plans and patients."

Minnesota is home to two of the nation's largest PBMs.

OptumRx, which is a division of Minnetonka-based UnitedHealth Group, handles pharmacy benefits for UnitedHealthcare and other health insurers. Prime Therapeutics, which is based in Eagan, works with a number of Blue Cross and Blue Shield health plans across the country.

In 2019, the Minnesota Legislature passed a law to regulate PBMs, which also negotiate prices that health plans pay drug manufacturers and pharmacies. These payments come through a complex system of rebates and/or administrative fees that has been criticized for lacking transparency .

The state Commerce Department last year levied a $500,000 fine against Rhode Island-based CVS Caremark, alleging the company used strategies not allowed under Minnesota law for steering consumers to its own pharmacies. CVS Caremark denied the allegations and did not admit liability.

The Oklahoma case dealt with self-insured employer health plans, which are primarily regulated by the federal government. The appellate court ruled that federal law preempted state rules that effectively pushed PBMs to create broader networks of pharmacy providers.

Oklahoma has asked for the U.S. Supreme Court to review the case. The brief filed Monday by Ellison and the other attorneys general supported the state's request.

Last year's ruling in the Oklahoma case conflicts with an earlier ruling where the court found "PBMs cannot evade state consumer-protection regulations under the cloak of ERISA pre-emption," the Minnesota Attorney General's Office said in a news release. ERISA is the federal law that governs self-insured health plans, where employers take the financial risk for the cost of medical claims.

"The result [with last year's ruling] is nationwide uncertainty for regulators, a corresponding increase in consumer harms and a substantial likelihood of continued litigation on the topic in light of the deep circuit split," the attorneys general wrote in their brief filed Monday.

Ellison's office said the Department of Commerce has reached settlements or issued cease and desist orders in 12 cases since Minnesota passed its law for PBM regulation.

Rite Aid announces plan to close 12 more Michigan stores: Here's the list

Rite Aid, one of the nation’s largest drugstore chains, plans to close 12 more Michigan stores, the company said in a court filing earlier this week as it seeks to exit federal bankruptcy protection.

The document filed Monday in U.S. Bankruptcy Court in New Jersey did not indicate when the stores would be shuttered or what would happen to the employees, but the nonprescription merchandise will be liquidated in "store closing" and "going out of business" sales.

The dozen Michigan stores account for less than 10% of Rite Aid's 186 stores in the state.

The Michigan stores are scattered throughout the state, with about half of them in metro Detroit, including:

  • 15411 Southfield Road, Allen Park
  • 107 Kercheval Ave., Grosse Pointe Farms
  • 37399 Six Mile Road, Livonia
  • 640 N. Milford Road, Milford
  • 1998 Biddle Ave., Wyandotte

The others slated to close are:

  • 3880 Wilder Road, Bay City
  • G4033 Fenton Road, Burton
  • 6026 Lapeer Road, Burton
  • 4519 Richfield Road, Flint
  • 936 E, Ludington Ave., Ludington
  • 2985 Main St., Marlette
  • 603 E. Savidge St., Spring Lake

Rite Aid, however, is not alone in closing stores. CVS and Walgreens also have said they plan to shutter hundreds of stores after decades of expansion, in part, as they also face increasing competition and opioid lawsuits.

The new Rite Aid closings fall on the heels of demands earlier this month by lenders that a proposed $20 million payout to company CEO Jeffrey Stein be curtailed before funding the company’s exit from bankruptcy, according to a Bloomberg news report.

In March, the Free Press reported a Rite Aid in Midtown Detroit was closing, likely because of theft. The move, the report said, was "disappointing for some longtime shoppers" but "not a big surprise to everyone." And there is concern, nationally, that closing so many drugstores will leave some communities without any.

Rite Aid — a Philadelphia-based chain founded in Scranton, Pennsylvania , as the Thrift D Discount Center in 1968 — announced in August it was filing for Chapter 11 bankruptcy after years of sustained growth under a mounting debt load and thousands of opioid lawsuits filed against the company.

The Rite Aid chain grew gradually, then quickly, by acquiring other drugstores.

It entered Michigan in 1984 with the purchase of a Grand Rapids-based chain, and pharmacies in Lowell, Flint, and Lansing.

In the last two decades, however, the chain has shed thousands of stores, going from more than 5,000 in 2008 to about 1,700 now. It also has struggled to be acquired by other chains. Deals with Walgreens in 2015, and with Boise, Idaho-based grocery chain Albertson’s in 2018, both fell apart.
